In order for you to participate in intercollegiate athletic
programs at Brown University, you are required to complete a health
questionnaire that will be reviewed by the Athletic Training
Staff. A web service has been developed to allow you to
complete this form from your home.
The health questionnaire site is secured. Therefore, you will be required to enter your Brown username and password to access the site. As a new student, you were assigned a username and temporary passkey, which you changed to a permanent password, when you applied to Brown. This username and password is the same one that is used to logon to Banner to check admission decision results, missing documents, etc. You will use this username and password to gain access to HealthQuest. If you no longer have that information, please contact the Brown Computing Help Desk for assistance by calling (401) 863-HELP.
To complete the health questionnaire you will also need your health insurance provider information for this upcoming school year. If you purchased the Brown Student Health Insurance, list it as your insurance carrier and your Banner ID as your insurance policy number.
The service will be available starting on August 1, and must be completed no later than August 15, 2009. If this form is not completed, you will not be allowed to participate in team practices or games. Information entered in the questionnaire is kept strictly confidential, and can be accessed by only a limited number of athletic training staff members.
